If you need an auto identification card, please contact Porter & Curtis at 866-871-5444. When speaking with a representative, mention you are calling from the Diocese of Metuchen and specify which location.
You will need to report the following information when requesting an Auto Insurance Card:
• Vehicle Information
Vehicle Identification Number
Year/Make/Model
Original Seating Capacity
• Registered Owner
• Effective Date
• Lienholder/Loss Payee Information (if applicable)
• Mailing address (if different from Registered Owner address)

• Copy of Agreement, Contract, Lease, Application or Town Permit in its entirety
• Name and Address of Certificate Holder
• Contact Information of person(s) requesting certificate (i.e. e-mail address, fax number)

The concept of Risk Control is comprised of two parts:
1) Loss Prevention
2) Loss Control
Loss Prevention is defined as the employment of mechanisms in an effort to prevent losses before they arise and can cause any damage. It is important to note however, that not every loss can be prevented, and therefore, mechanisms must be employed to mitigate the severity of those losses which could not be prevented. Some exposures to loss are simply not insurable; not all losses, even when covered, may be fully insured; and even when fully insured, loss experience will ultimately result in increased insurance premiums.
As such, it is better to try and avoid losses altogether, thus the need for effective risk control.

Claims related to Student Accident Insurance should be filed through its program administrator, Bollinger Insurance, and not through Sphere Risk Partners.
Student Accident Insurance coverage is purchased by every school in the Diocese for each enrolled student.
The plan is underwritten by Zurich American Insurance Company and is administered by Bollinger Insurance. The plan in effect provides coverage on an excess basis only, thus only those medical expenses which are NOT payable by your own personal or group insurance are eligible for coverage under this policy up to the limits.
