Before your petition can be accepted by the Tribunal, the following forms must be received.
Please keep a copy of all materials you submit to the Tribunal.
Here, you will state why you are challenging the validity of your marriage. A copy of the summary petition is sent to your former spouse at the very beginning of the process, informing him or her that an annulment petition is being made and what the petition alleges. We would prefer to receive this in typewritten form. The document is available in english and spanish.
This is a detailed story of your courtship and marriage. A thorough autobiography is essential to the Tribunal’s being able to reach a decision about the validity of your marriage. We prefer that you also submit this report in typewritten form. The documents are AUTOBIOGRAFÍA available in English and in Spanish (pdf).
The Witness Statement form should be given to four people who are willing to support your petition with written testimony. Present one to each of your witnesses and ask that they supply a statement following the outline suggested. This statement should be returned by the witness directly to the Tribunal. The most helpful witnesses are people who knew you and your former spouse during the courtship and for the duration of the marriage. Please check with your witnesses periodically to ensure that their statements are submitted to the Tribunal.
