Presently, we have a part-time opening for a Geriatric Case Manager at a residence for retired priests who will be responsible for planning, managing and coordinating the care of priests in residence. The following is a brief summary of the position:
Screen, assess priest’s cognitive functioning and his ability to care for himself; coordinate necessary medical services; explore programs and services that would be of benefit to the priest; determine when a priest needs a higher level of care and coordinate the next placement involving appropriate family members in that decision; communicate with the priest’s family, keeping them informed regarding any changes in the priests cognitive functioning or physical/ mental health.
Consult with staff as necessary to find effective ways to resolve conflicts as needed. Provide necessary support and education to other staff working in the residence.
Assist priests with basic assessments regarding any equipment or medication management/safety needs,
Be able to assess the priest’s ability to continue to be independent regarding keeping their doctor appointments, appointments for tests, and if they need transportation assistance. Also should be able to assess vision and hearing limitations broadly that may impact independence, along with cognitive assessments.
The ideal candidate will be highly organized, self-motivated, possess outstanding interpersonal and relational skills, and be able to maintain confidentiality.
Bachelor's Degree; or five to seven clinical related experience; or equivalent combination of education and experience.
