An Employee/Volunteer Application Form must be completed and returned to the parish/school background coordinator. The coordinator will ensure that basic background information on each employee/volunteer (name, address, e-mail address, date of birth, home phone number, title or position and optional social security number) is included on the application form. The background coordinator will identify if the employee or volunteer should be fingerprinted under the Policy.
Note: No one under the age of 18 should be fingerprint background evaluated.
The parish/school will provide the volunteer/employee with a MorphoTrust New Jersey Universal Fingerprint Form (IDG_NJAPP_020115_V2) containing the VRN for each location in Box #7 and Parish or School Location Agency Information (bottom of page). Separate forms are required for employees and volunteers.
Note: Teachers and school employees must be processed through NJ Department of Education Criminal History Review Unit Program.
Volunteer/employee should carefully read and follow all instructions to complete fingerprint process. The form must be presented to be fingerprinted. No exceptions. No variation of form will be accepted.
Appointment scheduling is available at www.bioapplicant.com/nj or through the Call Center. English and Spanish speaking agents are available at 1-877-503-5981, Monday through Friday, 8 a.m. to 5 p.m. EST and Saturday, 8 a.m. to 12 p.m. EST. The Applicant ID number, date and time of appointment and payment confirmation will be provided by the Call Center and must be recorded in the appropriate blocks on the bottom of the form. Payment is required at the time of scheduling. Visa, MasterCard, prepaid debit cards or electronic debit (ACH) are the only acceptable forms of payment. Reimbursement for background cost is at the discretion of the parish or school administration.
Note: Effective June 1, 2019 the cost for an Initial Volunteer Fingerprint is $24.05; the cost for an Initial Employee Fingerprint is $56.05.
Current identification must be presented at the time of printing. Examples of acceptable identification, issued by a federal, state, county or municipal entity are: (1) Valid U.S. State Photo Driver's License/Non Driver's License; (2) US Passport; (3) USCIS Permanent Resident ID Card (issued after 5/10/2010); and (4) USCIS Employment Authorization Card (issued after 5/10/2010).
Appointments may be cancelled or rescheduled through the website or Call Center before 5 p.m. of the business day prior to the scheduled appointment. A fee of $10.70 will be charged for those who do not cancel or reschedule. The $10.70 fee also applies to applicants who fail to appear; are turned away from the printing sites due to their inability to present proper identification; or who fail to present Universal Form IDG_NJAPP_020115_V2. State and federal search fees will be refunded.
After fingerprinting, the MorphoTrust form and receipt should be photocopied and provided to the parish/school background coordinator as proof of processing.
If needed, MorphoTrust will notify any individual that requires re-fingerprinting because of any computer error. A 90-day time period is enforced by MorphoTrust for completion of re-fingerprinting. Any further delay will result in applicant being charged a new applicant fee.
The New Jersey State Police and Federal Bureau of Investigation will conduct a background search and submit the results to the Office of Child & Youth Protection.
The Office of Child & Youth Protection will provide a copy of State Police paperwork to the parish/school background coordinator.
In the event of a Non-Recommendation Notice of background of volunteer/employee, the background coordinator, in coordination with the pastor/principal, will ensure that said volunteer/employee is removed from any position that puts said person in contact with minor children until the matter is resolved by Office of Child & Youth Protection as set forth in diocesan policy.
