The Name Check (SBI 212B) Form is a New Jersey State Police Background Check that allows the Diocese the ability to submit a request to run the name of an individual in order to identify if there is any criminal history record information. Each submission costs $20 (subject to change) with the exception of submissions for volunteer work, which costs $12 (subject to change).
The Diocese has formerly used this form to conduct every three year rechecks after the individual had completed an initial fingerprint upon starting the process of meeting Diocesan Safe Environment Policy. Effective as of June 15, 2023, the Diocese no longer require rechecks. The individual will now be continuously monitored through Selection.com/FASTRAX®'s Quarterly Watch Program, which will provide continuous monitoring of the individual for the duration of their employment and/or volunteer service working with children and/or vulnerable adults. However, the 212B Form - Name Check Only Request may be used at the discretion of the Office of Child & Youth Protection depending on the circumstance for clearance purposes. Upon determination that a Name Check is needed, the Diocese will submit the individual through to the State Police and will cover the cost of this background check.
If the individual is submitted for a Name Check, the individual will receive an email from the State requesting the authorization to conduct the Background Check. If this background check is not authorized, the Diocese reserves the right to deny the individual the ability to working with children and/or vulnerable adults until they authorize the background check and comes back cleared by the NJ State Police.DOE Background Check Instructions CRIMINAL HISTORY RECORD CHECK INSTRUCTIONS |
